When you send us a message or request a visit, we collect what you type into the form: your name, email address, phone number, how and when you would like us to reply, and your message. Please keep medical details out of the form. Your health history goes in the forms in our secure patient portal.

This website does not run advertising trackers. Our hosting service keeps basic server records, such as the time of a visit and the type of browser, so the site stays secure and working.

How we use it

We use what you send us to:

• Reply to your message and schedule your visit
• Send appointment reminders by the method you choose
• Check your insurance benefits and bill for your care
• Meet our legal obligations

Who we share it with

We keep your records private and share them only as the law allows. We do not sell your information. We share it with people who help care for you or run the practice, such as a dental laboratory, your insurance plan or a lender you choose, and only what each needs.
